Notes:

The chords in parenthesis are for the key change on the last chorus out.
For solo (acoustic) guitar I would recommend playing "full" chords, but 
for electric guitar, I play power chords throughout. I also use a low gain 
setting on the distortion channel with the volume on the guitar at around 7, 
and for the last chorus with the key change I turn the guitar volume all the 
way up.
